چکیده
A fundamental challenge in conduction system pacing [CSP] is determining appropriate lead deployment. With either fixed core or stylet driven leads, the input number of turns may not match resultant rotations achieved by helix. To date, no validated ex-vivo experimental model has been developed to enable quantitative assessment ratio helix rotation. develop a quantitatively describe CSP tip tissue response curves. Sixty-four fresh (<48h) ovine right ventricular septa were excised. Four different leads (1 (A), 3 stylet-driven (B,C,D)) into septal myocardium using support via custom-engineered jig allowing control rotation and measurement Leads inserted 15 applied proximally on lead, with tissue-interface characteristics observed at each 90° increment high resolution photography. Output (helix) vs curves [RRC] assessed automated segmented linear regression. RRC models statistically analysed slope (to characterise output response) breakpoints abrupt torque transfer [AAT]). total 3840 observations ¼ turn proximal recorded. 16 repeated RRCs created for type. Pacing the human cardiac conduction system during open-heart surgery.
Specialized fiber electrograms were recorded from selected sites along the His bundle and bundle branches during open-heart surgery in 26 patients. The heart was then paced from the same recording sites. Stimuli applied to the proximal His bundle recording site always resulted in His bundle pacing, which was characterized by a stimulus artifact-to-QRS interval which equaled or very nearly equal...

متن کاملReceptor Editing Occurs Frequently during Normal B Cell Development
Allelic exclusion is established in development through a feedback mechanism in which the assembled immunoglobulin (Ig) suppresses further V(D)J rearrangement. But Ig expression sometimes fails to prevent further rearrangement. In autoantibody transgenic mice, reactivity of immature B cells with autoantigen can induce receptor editing, in which allelic exclusion is transiently prevented or reve...
متن کاملPlacental transfer of darunavir in an ex vivo human cotyledon perfusion model.
Placental transfer of the HIV protease inhibitor darunavir was investigated in 5 term human cotyledons perfused with darunavir (1,000 ng/ml) in the maternal to fetal direction. The mean (± the standard deviation [SD]) fetal transfer rate (FTR) (fetal/maternal concentration at steady state from 30 to 90 min) was 15.0%±2.1%, and the mean (±SD) clearance index (darunavir FTR/antipyrine FTR) was 40...

Placental transfers of the HIV nonnucleoside reverse transcriptase inhibitor rilpivirine were investigated in 8 term human cotyledons perfused with rilpivirine (400 ng/ml) in the maternal-to-fetal direction. The mean fetal transfer rate (FTR) (fetal/maternal concentration at steady state from 15 to 90 min) was 26% ± 8% (mean ± standard deviation), and the clearance index (rilpivirine FTR/antipy...
